Proof of Eligibility: REQUIRED for NONPROFIT ORGANIZATIONS (ARTS and SOCIAL SERVICE). Proof is available online. Navigate to the Office of the Illinois Secretary of State site, enter your organization's name in the search box, and submit the request. The screen that follows should be the "Corporation File Detail Report." For nonprofit status to be accepted, it must include the file number and be active. Take a screen shot and create a PDF. Upload the PDF.
