2022-2023 Agreement to Little Cadets Program Requirements

I understand I am submitting enrollment in the Little Cadets Preschool Program as my preschool provider for the entire 2022-2023 school year. I will provide transportation to/from the program. I will ensure I am on time to pick up my child each day. I understand if I am late picking my child up then I will be subject to an additional charge. I will remain current with all my fees (please refer to Financial Agreement Formregarding amount due and due dates). I will abide by all Little Cadets Preschool Program requirements.

2022-2023 Financial Agreement Full Day Form
The preschool fee for our Little Cadets program is 10 months and follows the academic calendar of Mill Creek Community Schools. Payments for Little Cadets Preschool can be made in the form of check or cash directly to the Mill Creek East Elementary front office or parents can set up online payment on through their Parent Portal. By initialing below, I have indicated the payment option I have selected for the 2022-2023 school year for my child. Monthly payments will be due and made on the 15th of each month, beginning on August 15th, 2022 and ending on May 15, 2023. Payment options are as follows: Extended day option (6:30am-6pm) is 10 payments at $620/month. Preschool only option (9am-3:45pm) is 10 payments at $410/month. Partial Day Preschool option (9am-1:30pm) is 10 payments at $370/month. Preschool with Extended Day AM or PM (not both) is 10 payments at an additional $110/month. In the event that any of these dates fall on a day the MC School Corporation and/or Little Cadets Preschool is closed, the payment will be due on the school day prior to the day off.

Payments will be made as indicated above. If at any time I would like to change the frequency of payments, it is my responsibility to complete and submit a new Financial Agreement Form. Late fees of $1.00 per minute (dollar amount increases as the number of late times increase) for each child not picked up by closing will be added to my account. If a personal check submitted for payment is returned for non-sufficient funds or account closure, I understand I am responsible for the full amount of the check plus a $20 returned check fee. If I choose to withdraw my child at any time, I will submit a two week notice in writing, and a final fee will be calculated. If my account becomes past due on day 30, I understand a $35 late fee will be assessed. If the account is delinquent after two months, I understand services will be stopped and the child will be unenrolled from the Little Cadets Preschool program until payment arrangement has been made. If payment is not arranged and services are terminated, my account will be turned over to a collections agency and the full amount plus collections fees will be owed.

I understand that I will still owe payment in the event of closure due to weather or school-based emergencies, child illness, or family vacation.
