Full payment is due January 23, 2026. Disclaimer: To attend a trip, the student must be current in school tuition and have no outstanding balances on the school account. By providing your signature, you confirm that you give permission for your student, listed above, to attend an SCLA Mission Trip. You understand that trip cancellation insurance will be purchased by SCLA for all flights. You also understand that in the case of a flight cancellation, flight refunds are at the discretion of the airline, and some refunds may be in the form of credit towards a future flight instead of a cash refund. In the case that no cash refund is provided by the airline, SCLA cannot provide a cash refund to participants for canceled flights. You acknowledge on behalf of yourself or your minor child that you assume all risk of personal injury, death or disability which may result from participation on this trip. You certify that you have adequate health insurance to cover any injury or damage caused to yourself or your minor child, or else agree to personally bear the cost of injury or damage. You agree to defend, indemnify and hold St. Croix Lutheran Academy and its representatives harmless from any and all losses, liabilities, claims, damages or other financial harm incurred while on an SCLA Mission Trip including reasonable attorneys' fees, as a result of any actions of you or your minor child as well as any injury or damage caused to any other person during your participation and activities on an SCLA Mission Trip. You also authorize and consent to the use of your student's voice, photograph and likeness covered by this Agreement by St. Croix Lutheran Academy without reservation or limitation. You agree to receive no compensation for this use, and property rights to any photographs or video material produced or prepared by St. Croix Lutheran Academy shall vest in and remain with St. Croix Lutheran Academy.
