MENTOR REPORT FORM
Ordination Candidate

This Mentor Report Form documents each Meeting held between a Candidate for Ordained Ministry (also known as "Mentee") and their Mentor. The Mentor provides ongoing nurture and direction through monthly/bimonthly Meetings with the Mentee during their Ordination journey.

This Form to be completed by the Candidate's Mentor. Once submitted, the Form will be emailed to the Candidate for final review and approval. Once approved, this Form is remitted to the Mid-America Regional Office in Springfield. All Mentor Meeting Reports will be stored in the Candidate's clergy file to be read and reviewed by the Members of the Regional Commission on the Order of Ministry (RCOM).
