Ordination Candidate - Appendix 8

This Mentor Report Form documents a conversation between a Mentor and their Mentee as a Candidate for Ordination through the Seminary or Apprentice Track. In partnership, a Mentor supports their Mentee during the journey of Ordination through monthly or bimonthly Meetings.

All Mentor Meeting Report Forms are stored in the Candidate's clergy file to be read and reviewed by the Members of the Regional Commission on the Order of Ministry (RCOM).
