  • The DeWayne B McCamish Lifetime Achievement Nomination Form

    The Lifetime Achievement Award, established in 2008, is presented to honor orthodontists and non-orthodontists who have contributed outstanding lifetime service to the Specialty of Orthodontics.
  • NOTE:  The distinction between the Oren Oliver Award and the Lifetime Achievement Award:  The Oliver Award is given to a member whose service has been to the SAO; the Lifetime Achievement Award is given to a member whose service has been to orthodontics in general.

     

    The deadline to turn in a nomination is February 21st.

    * After discussion at the Interim Board Meeting, the SAO Board of Directors will vote by written ballot to determine the recipient. A simple majority of the SAO Board is necessary for selection of the Award.

    • Criteria 
    • 1.  The achievements of the recipient rise above the ordinary and are far beyond the expectation of service and leadership to the Specialty of Orthodontics

      2. The work of the recipient has advanced the mission of orthodontics, which is quality patient care.

      3. The recipient has shown dedication and commitment to orthodontics over a significant part of his/her professional life.

       

      * A candidate must be nominated by a SAO member in good standing.
