• The Citizenship Award Nomination Form

    The SAO Citizenship Award, established in 1989, recognizes SAO members whose voluntary service activities exemplify dedication to community, democratic standards, and/or spiritual values, thereby creating favorable public opinion about orthodontists.

    • Criteria 
    •  * Component organizations determine eligible candidates. 

      • Nominations must come from the members at large of a component organization or the Leadership of the component organization.
      • Nominees are presented to the SAO Board of Directors for approval.
      • Each component may nominate a member for the Citizenship Award although the Award does not have to be given annually. If more than 3 nominations are received, the President will appoint an Awards Committee to make recommendations to the Board of Directors.
