Artist Commission Form

This form is to accept or decline a commission agreement between your company and WSY for manufacturing of apparel and accessory products based off of your works of art at a commission rate of 30% of the profit.

Do you give We See You Art& Poetry permission to reproduce select artworks for manufacturing by King's Prints Production, for a 30% commission rate to be paid out based on the selected payment terms.
