I hereby request an official change in classification at Adams State University. I understand that I am limited to one such request per fiscal year (Summer, Fall, Spring) and that my choice of classification reflects:

1) my primary purpose(s) for enrolling at ASU during the current fiscal year;
2) the appropriate ASU tuition for one with such purpose(s); and
3) the appropriate level of courses I may enter.
Classification options are as follows:

Undergraduate Special (US)
- Purpose: Pursuing undergraduate courses exclusively
- Level of Courses: 1-499
- Tuition: Undergraduate
Graduate Special (GS)
- Purpose: Pursuing graduate and/or undergraduate courses (not seeking MA degree)
- Level of Courses: 1-599
- Tuition: Graduate
