I agree to attend the facility under the following terms of entry and I accept full responsibility for my actions.

I am aware that signing this form affects my legal responsibilities.

In consideration of participating in the activities at the facility (and except to the extent that the same may be precluded by statute) agree to release and indemnify AUSA Hoops and its Operators, Owners, Franchisees, Staff, Contractors and Franchisor as follows;
1. I participate in the activities at any AUSA Facility at my own risk and responsibility.
2. I release, and hold harmless AUSA Hoops, its servants and agents, from and against all and any actions which may be made by me or on my behalf or by other parties for or in respect of, or arising out of injury, loss, damage or death caused by me, my property whether by negligence, breach of contract or in any way whatsoever.
3. I accept all responsibility for any injury, loss or illness that may arise from my use, classes or any other product or service used within the gym. I will not hold the gym responsible in any way.
