USTA League Grievance
Prior to submitting a grievance, captains and coordinators should review and be familiar with USTA Mid-Atlantic Adult League Grievance and Appeal Procedures as well as Section 3.00 of the USTA League Regulations, with particular attention to Sections 3.03A, 3.03B, 3.03D, 3.03E and 3.04.

Please note, anonymous statements are not accepted and video submissions of any kind are not permissable in the USTA grievance or appeal process. All information submitted pertaining to a grievance or grievance appeal must be in writing in order to be shared with the Committee and all parties involved. All files and attachments uploaded to this grievance form must be in PDF format. Please make sure that all information is included as your grievance form will be considered complete upon submission.
