I understand that in my participation, exercises involved may include aerobic, and and strength training activities.I understand that with my voluntary participation, I will be given a fitness training plan that could bring improvement of my cardiorespiratory (heart and lungs), musculoskeletal (muscles, joints, and bones), and overall body fitness.
I understand that during the conduct of exercise, the reaction of my heart, lungs, and blood vessels, etc., cannot be predicted with accuracy, thus risks may arise due to adverse changes of that may lead to heart attack, high blood pressure, or stroke.I understand that during training, injuries may arise such as muscle, ligaments, joints and tendons; and modifications will be presented to prevent or minimize these occurrences. I understand that I shall be solely responsible, and that 3024 will not be held liable for any damages and inquires arising from the fitness training program.
I am aware that all information obtained by the fitness center from me, such as fitness records, personal information, among others, shall be treated as confidential information and that 3024 holds these information as privileged information. These information shall not be disclosed nor shared to anyone without my prior written consent.I understand that the information acquired by 3024 may be used for statistical purposes or others so long as such information will not disclose the identity of the person.I understand that the workouts are recorded for replay purpuses, and or marketing of 3024 inititives. I am ok with recorded workouts.
