I agree that in the event of injury or illness, I will take reasonable steps to contact the club, and I give permission for the club to administer any appropriate or necessary medical attention. I understand that it is my responsibility to keep the club updated with all relevant information including medical and contact details. I give consent to Limitless Sports to maintain records and information relating to my child in confidential electronic documents and in club information folders for administration purposes. I understand that I have a right to see this information if I so desire, providing I give appropriate notice to the club. I consent to the club using appropriate images and photos/video of my child for purposes relating to the promotion and marketing of Limitless Sports. I have read a copy of the club’s constitution & codes of conduct HERE. I agree to abide by them and understand that any breaches of the club’s rules and policies will be dealt with in accordance with the club’s disciplinary procedures that are I force at the time.
Following submission of this form, you will be sent the clubs bank details by whatsapp or email to make payment & complete registration. Your childs space will only be confirmed once payment is complete. Following completion of payment, you will then be added to a temporary WhatsApp group where further information may be sent.
