All parents should write their children's medication in the medicine book, or by email, inform the Center. Medicines must be submitted to the person on duty. No medicine may be left in the child's suitcase.

Ouers aanvaar dat alle redelike voorsorg getref sal word om die veiligheid en welsyn van hulle kind te verseker en dat hulle aanspreeklik gehou sal word vir die betaling van mediese en/of hospitaalrekeninge waar van toepassing, indien enige besering opgedoen word. Ouers verleen hiermee toestemming aan die naskool dat hulle kind, in geval van `n noodgeval, na `n mediese dokter vervoer kan word, nadat hulle telefonies in kennis gestel is.

Parents accept that all reasonable precautions will be taken to ensure the safety and well-being of their child and that they will be held liable for the payment of medical and/or hospital bills where applicable, if any injury is sustained. Parents hereby grant permission to the after-school that their child can be transported to a medical doctor in case of an emergency, after being notified by phone.

'n Registrasiefooi van R100 per kind is betaalbaar met inskrywing en is nie terugbetaalbaar nie.

A registration fee of R100 per child is payable upon entry and is non-refundable.

Ek, die ondergetekende, ouer / wettige voog van die genoemde leerder wat as sodanig ingeskryf en aanvaar word deur Lighuis Huiswerk & Projeksentrum, onderworpe aan die bepalings wat hierin uiteengesit word, vrywaar die Sentrum en die Eienaars en Personeel vir enige verliese of skade in die algemeen, maar dit kan voorkom, dat ek as ouer / wettige voog van bogenoemde leerder kan ly as gevolg van enige gebeurtenis waardeur die leerder betrokke kan wees, hetsy as die oorsaak of lydende party, terwyl hy/sy aan enige Sentrum verwante aktiwiteit deelneem. In die besonder magtig ek dat die voorgenoemde leerder betrokke kan wees by alle uitstappies wat gedurende skooldae/skoolvakansies deur hierdie sentrum onderneem word as deel van sy/haar leerervaring en, waar van toepassing, stem ek saam dat hy/sy die vervoer wat deur Lighuis Sentrum gereël word, vir sodanige uitstappies mag gebruik. Ek vrywaar ook Lighuis Sentrum en die Eienaars en Personeel vir enige skade of verliese wat ek as ouer / wettige voog van bogenoemde leerder onder sulke omstandighede mag ly en vrywillig die risiko's wat daarmee gepaard

I, the undersigned, parent / legal guardian of the mentioned learner who is enrolled as such and accepted by Lighthouse Homework & Project Centre, subject to the terms set out herein, indemnify the Centre and the Owners and Staff for the time being for any losses or damages in general, however they may occur, that I as parent / legal guardian of the above learner may suffer as a result of any occurrence whereby the learner may be involved, whether as the causing or suffering party, whilst participating in any Centre-related activity. In particular, I authorise that the aforesaid learner may be involved in all excursions undertaken by this centre during school days/school holidays as part of his / her learning experience and, where applicable, I agree that he / she may utilise the transport arranged by Lighthouse Centre for such excursions. I also indemnify Lighthouse Centre and the Owners and Staff for any damages or losses that I as parent / legal guardian of the above learner may suffer under such circumstances and voluntarily accepts the risks associated therewith.
