    Participation Form

  • The annual Wine & Chocolate Walk will be held on Sunday, May 17, 2026 from noon to 5 p.m. All tickets are $35 each. To purchase your ticket(s) please continue below or call (440) 984-6709 with any questions. Only 350 tickets will be sold and all participants must be 21 or older. You must have ticket the day of the event or you will not be able to participate, no exceptions. Thank you.

    The day of the event:

    1. Bring your ticket. A physical ticket must be presented to participate.

    2. Each ticket holder must be present with their ticket in order to get their bag and information to participate.

    3. Registration will take place on the loading dock behind the M.S.A. Community Center (255 Park Ave.).

    4. Registration will take place between 11:30 a.m. and 1 p.m. Once registration is over you will no longer be able to participate. 

    5. Ticket(s) need to be picked up one week before event. You may send someone else to pick up your tickets. They will need to know your name, bring their driver's license and sign for the ticket(s).

    In order to decrease the line before the event, please pick up your tickets at the Main Street Amherst Community Center Monday-Friday 9am to 1pm. We cannot send tickets in the mail. Someone else may pick up your tickets for you. Please give the office a call at (440) 984-6709 to make sure someone is available for pick up or to let us know who is picking up your tickets.  We will also email possible evening pick up dates when determined. We appreciate your help with this! By signing above, you understand that there are no refunds after your purchase. Thank you.
