Walkovers notified with over 24 hours' notice from the start of scheduled game time will be charged at $95

Walkovers notified with less than 24 hours from start of scheduled game time will be charged at $110

Walkovers given without notification will be charged $190.

Amount owing will be strictly based upon submission time of this form.
