Please complete the following form to notify of any walkovers. This is the only accepted form of notification.

Walkovers notified with over 24 hours notice from start of scheduled game time will be charged at $90

Walkovers notified with less than 24 hours from start of scheduled game time will be charged at $110

Walkovers given with no notification provided will be charged at $180.

Amount owing will be strictly based upon submission time of this form.

Senior competition forfeits must be notified via Teammo.
