Shirt sizes are general options and seem pretty true in size/fit. Adult sizes, from small to 4XL.

Hats will be the flex style, in sizes of Small/Medium, Medium/Large, Large/XL.

For jersey numbers, usually you'd get first option of number, but if players request same then one gets their 2nd option. 3rd option is unlikely to be needed, but please select something different just in case. So DO NOT REPEAT any selections or we'll just select a default number option if needed.

PREFERRED NAME (First OR Last name for back of shirt) - Names must be actual name used. Variations like 'Nick' for 'Nicolas', 'JT' for 'John Thomas' are okay, but no unrelated nicknames please. We won't approve other versions, and would then use the 'Registered' First Name instead.
