    It will take approximately 20 minutes to complete this form.

    Due to overwhelming request, our schedule is currently very full and we may not be able to schedule your appointment as soon as you would prefer.

    Please read the terms and conditions below and sign at the end of the form. Doing so states that you have read and agree to our terms and conditions.
  • Safety

    Safety comes first for everyone during the grooming process for both our groomers and your pet. Please inform us prior to grooming if your pet has bitten someone or has past aggression issues. We are able to handle difficult pets, but only if we are aware and prepared. If a dog is very traumatized or stressed about the grooming process, we will stop the service and recommend you speak with your veterinarian about medication to help calm your pet so he can be groomed safely. You will only be charged for the amount of work accomplished to that point.

     

    Veterinary Care

    We make every effort to ensure your pet’s grooming is pleasant and safe. If your pet becomes ill or is injured and the services of a veterinarian are required, you hereby give us permission to obtain treatment if you are not available to do so.

     

    Pick up Times

    We kindly ask that you please pick up your dog shortly after her groom is completed. We will contact you as soon as your dog is ready to go. Any dog left here for more than 1 hour after the phone call is made for you to pick up will be assessed an additional care fee of 10 dollars per hour.

     

    Senior Dogs & Dogs With Health Issues

    Grooming procedures sometimes can be stressful, especially for a senior pet or a pet with health problems. Grooming may expose hidden medical conditions or aggravate existing ones during or after the grooming process. Because senior pets and pets with health problems have a greater chance of injury, these pets will be groomed for cleanliness and comfort, in styles that will not add to their stress. Dog Days Daycare will not be responsible for accident or injury to an elderly or health compromised pet during their grooming process. We reserve the right to stop the grooming process at any time if we feel it is in the best interest of the dog.

     

    Matted Coat

    Removing a heavily matted coat includes risks of nicks, cuts, or abrasions due to warts, moles, or skin folds trapped in the mats. Heavy matting can also trap moisture near the pet’s skin allowing mold, fungus, or bacteria to grow, revealing skin irritations that existed prior to the grooming process. After effects of mat removal procedures can include itchiness, skin redness, self-inflicted irritations or abrasions and failure of hair to re-grow. Shaved pets are also prone to sunburn and should have sun screen applied frequently or should be kept out of the sun until the hair grows sufficiently to protect the skin. In some cases pets may also exhibit brief behavioral changes. It is our policy to de-mat a dog's coat only once in order to make the pet comfortable and to start re-building the coat. De-matting can be painful for your dog and unpleasant for both dog and groomer alike. After the de-matting process you should be able to maintain the dog’s coat between grooming appointments. DDDC reserves the right to refuse grooming if your dog becomes heavily matted again after the initial de-matting. Those pets with such heavy matting will have to then be shaved. We will not attempt to save the coat the second time.

     

    Shaving Pets With Thick or Double Coats

    Shaving a heavily coated pet during the summers is NOT RECOMMENDED. In both hot and cold weather, a dog’s coat helps regulate body temperature, provide insulation and protection from the sun. Coats that are shaved off, particularly on double coated dogs such as the Husky or Shepherd may grow back dull, patchy, or not at all. If you are looking to cut back on shedding or keep your pet cool, the best course of action is frequent brushing to remove loose hair and allow air to move freely through your dog's undercoat. A good bathing and de-shed program will help tremendously. Please contact us for more information.

     

    Refusal To Groom Aggressive Pets

    Dog Days Daycare has the right to refuse grooming services for aggressive pets at any time. It is the customer's responsibility to inform the groomers of any known past aggression issues, large or small. If your pet becomes overly aggressive or bites at any time during the grooming process, Dog Days Daycare has the right to end the grooming and call for the pet to be picked up. Owner will be responsible to pay for whatever services that were able to be rendered.

     

    Cancellations or Missed Appointments

    On rare occasions, Dog Days Daycare may need to cancel your appointment due to weather, road conditions, equipment failure, etc. Every effort will be made to contact you in advance. If you cannot keep your appointment, please contact us at least 24 hours in advanced so we may fill your spot. Please be prompt for your appointment to avoid delays and inconvenience to others.

     

    Returned Check

    All grooming fees must be paid at the time services are rendered. Checks returned due to insufficient funds will be subject to a $35 returned check fee.

     

    Photographs

    Dog Days Daycare reserves the right to take pictures of your pet before and after grooming for our website, advertising, and/or social media.
