    Questionnaire - Industry Personality
  • This questionnaire has been developed to solicit information about the nominee and his/her contributions to enable a fair assessment by the Award’s Technical Committee in line with the Awards criteria. The reference period for the Awards is 2018 – 2021. Supporting documentation such as financials, taxes, Board minutes extracts and other compliance evidence should cover the reference period.  

    Nominees do not have to be members of ACGN. Vetting of nominations and shortlisting of candidates will be done by the Awards Technical Committee of ACGN which comprises professionals from both public and private sector institutions.

    Candid responses to the following questions are of importance to the success of the ACGN Corporate Governance Excellence Awards. Nominees/Candidates are therefore advised to ensure full completion of the questionnaire. Incomplete questionnaires will not be processed.

    NB: All information provided will be treated as confidential and used solely for the purpose of the Awards.

    The Awards ceremony is scheduled for 18th November 2022.

  • For all categories of the awards, the individuals would be assessed on four Corporate Governance outcomes including;

    1. Ethical Culture
    2. Good Performance
    3. Effective Controls
    4. Legitimacy

    This section seeks to establish the distinguished contributions and qualities of individuals who have excelled in good Corporate Governance practices in their organizations/institution, country and Africa

  • F. SPECIFIC REQUIREMENTS FOR THE VARIOUS CATEGORIES OF AWARDS

  • 1. Influential Director of the Year

    NB: In addition to the specific requirements stated here, applicants for the Influential Director of the Year category must score high under general individual requirements question number 2. That is, description of "your strategic leadership which led to the achievement of social, environmental, and economic success for your institution, country and the African Continent at large."

  • 2. Innovative Director of the Year

    NB: In addition to the specific requirements stated here, applicants for the Innovative Director of the Year category must score high under General Individual requirement number E8. i.e., “your good management ability, proficiency, and customer service ability performed in such a manner that the employees and/or your team performed significantly above those with similar duties, and that the results of these efforts were distinctly beneficial to your organization, country and Africa.”

  • 3. Up and coming Director of the Year

    This Award is limited to individuals who are not more than forty (40) years old. The aim is to encourage the youth to achieve excellence while they are still young. NB: Applicant for this category must provide evidence of age by a copy of birth certificate or any other equal proof.

  • 4. Overall Director of the Year

    NB: Applicants who excel in satisfying all the twelve (12) requirements under the general requirements would be shortlisted for this category. The emerging overall Director of the Year is the applicant who excelled most.
  • G. CRITICAL REQUIREMENTS AND SUBMISSION

  • 1.      All individuals nominated must be prepared to undergo vetting where necessary.

    2.      All individuals nominated must confirm that they or their organization are not under any investigation by any Regulatory Authority or law enforcement body.

  • H. SELECTION PROCESS

  • 1.      After the launch of the Awards, nominations would be officially opened and application forms placed on the Network’s website for applicants to fill and submit as per instructions on the website.  

    2.      The Awards Technical Committee would sit to review all submitted questionnaires and carry out relevant due diligence on applicants. The Awards Technical Committee would present the applicants in order of merit, to the Executives of the Chairs Forum for further action.

  • Should be Empty:
