We use progress reports, report cards and attendance records to track students' progress and provide support as needed. Please read carefully and sign below permitting us to request records directly from your scholar's school.

I hereby grant Buffalo Prep permission to access any information relating to my child's academic progress; including but not limited to report cards, progress reports and scores on standardized tests.

Requirement for application to be completed:
- First 3 pages of 2023 taxes
- A 2021-22 final report card or a current report card for 2023-24 school year progress reports will NOT be considered

Proof of Income-Submit the first three (3) pages of the 2023 federal tax return(s) for the parent/ guardian financially responsible for scholar.
