  • DIRECTIONS:

    There are two options for submitting your application. The applicant can submit a filled-out application & required site plans by clicking on the submit button at the end of the application. Option 2 you can fill out and download the application at the end of this online form, and email Mike Kieliszewski at mjk-kelly@charter.net along with all required site plans.

    Option 1 Submit Typed Form (preferred method):

    1. Please complete the online form by clicking the next button below. Once you complete the form, click the submit button. This will submit the required site plans and application to the Soil Erosion Officer. Additionally, you will have a chance to download a typed-out, filled-in copy of your application and save it to your computer. This option will be shown after you click the submit button on the last page of the form and you will then be taken to the thank you page where you can download the typed-out, pre-filled application. By default, the application will automatically be emailed to you as well. 

    Option 2 Download Filled in Form to Email:

    1. If you prefer, you can download a typed-out application after you complete the form and save it to your computer by clicking the green button below that says email form only at the end of the application. Once you click the green button, you will be directed to the thank you page where you can download the form directly to your computer and email the Soil Erosion Officer at mjk-kelly@charter.net with the filled-out application and required site plans.

    Commercial projects, greater than one acre, require sealed, engineered site plans with runoff calculations, and either a signed letter of acknowledgment or a letter of credit. If the inspector feels that it is necessary to have a professional engineer review the plans submitted to the County, all review costs will be paid for by the developer.

    Utility and Corridor Development
    To include but not limited to: underground cables, above ground major transmission cables, water mains, and sewers.

    * Additional Site Inspections for any permit, beyond three, will be charged at a rate of $50.00 per visit and will be billed to the landowner at the expiration of the permit.

    Any projects, one acre or larger, with a stormwater collection, or discharge point may require a stormwater operator for the duration of the project. Any project five acres or larger shall require a certified stormwater operator for the duration of the
    project.

  • General Conditions

  • In accordance with Rule 1709 promulgated under the authority of Part 91, Soll Erosion and Sedimentation Control, of the Natural Resources and Environmental Protection Act, 1994 PA 461, as amended, and in addition· to the information on the attached plan(s) and special conditions, the following general conditions apply to the earth change authorized by this permit:

    • D esign, construct and complete the e arth change in a manner that limits the ex posed area of  disturbed land for the shortest period of time.
    • Remove sediment caused by accelerated soil erosion from runoff water before it leaves the site  of the earth change .
    • Temporary or permanent control measures shall be designed and installed to c onvey water  around, though, or from the earth change at a non - erosive velocity.
    • Install temporary soil erosion and sedimentation control measures before or upon  commencement of the earth change activity and maintain the measures on a daily basis. Remove t emporary soil erosion and sedimentation control measures after perman e nt soil  erosion measures are in place and the area Is stabilized. ('Stabilized" means the establishment of  vegetation or the proper placement, grading, or converting of soil to ensure It s resistance to son  erosion, sliding , or other earth movements).
    • Complete permanent soil erosion control measures for the earth change within five calendar  days after final grading or upon completion of the final earth change. If It Is not possible to  perm anently stabilize the earth's change, then maintain temporary soil erosion and  sedimentation control measures until permanent soil erosion control measures are in place and  the area is stabilized.
