    Permission to Conduct, Write & Submit Documentation

    I, Sherina James (name of trainee) am providing you, ("Practice Client") with the following information so you can make an informed choice about your decision to grant me permission to conduct a practice EFT session with you (the "Session" You understand I am enrolled in Tap Your Power, LLC's, DBA EFT Tapping Training Institute (the "Institute") mentor program to obtain accreditation from EFT International as a Certified EFT Practitioner (the "Program" The Program requires I conduct EFT practice sessions with individuals and submit written session notes of individual Sessions or case studies for multiple Sessions (hereinafter collectively referred to as "Session Documentation") to the Institute's Program Director(s) for the sole purpose of the Program Director(s) evaluating my level of competency as an EFT practitioner and to determine if I have met the criteria established by the Institute to complete the Program.

    You understand in the Session I will be using Emotional Freedom Techniques or EFT, an innovative energy-oriented method based on the emerging field of Energy Psychology. EFT seeks to address imbalances within a person's energy system, as well as the energetic influence of thoughts, beliefs, and emotions in the body. The prevailing premise of EFT is that the cause of all negative emotions is a disruption in the body's energy system. It is believed that EFT helps clear these disruptions, thereby restoring the flow and balance of the body's electromagnetic and more subtle energies. EFT uses the ancient Chinese meridian system with a gentle tapping procedure which stimulates designated meridian end points on the face and body while saying specific phrases and focusing on issues of emotional intensity in order to release the intensity and reframe the issues.

    Although EFT appears to have promising health benefits and there is a growing amount of valid scientific research indicating that EFT meets the criteria to be considered an evidence-based technique, EFT has yet to be fully accepted by the mainstream Western academic, medical, and psychological communities. Therefore, EFT is currently not officially identified as an evidence- based technique and may be considered experimental. Since EFT's risk and benefits are not fully known, you agree to assume and accept full responsibility for any and all risks association with participating in the Session(s) and using EFT.

    You understand the Session Documentation is solely for training purposes only and will be submitted electronically to and reviewed by the Program Director(s) for the sole purpose of evaluating me as an EFT practitioner. The Program Director(s) will not be providing any evaluation or assessment of you, the Practice Client. Further, you understand that although I and the Program Director(s) will make every effort to protect your privacy and confidentiality with respect to your personal information contained in the Session Documentation, it's not possible to guarantee the confidentiality and privacy of such information transmitted electronically.

    By signing this document, you grant to me permission to conduct the Session and write the Session Documentation of any Session that you participate in as a Practice Client and consent and authorize the Program Director(s) to review the Session Documentation and discuss it with me.

    You understand that your participation as a Practice Client is strictly voluntary, at your own risk, 

    and that you freely choose to participate in your Session(s You agree to forever fully release and hold me harmless from and against any and all claims or liability of whatsoever kind or nature which you might incur as a result of your voluntary decision to participate in the Session(s) as a Practice Client. Further, you agree to forever fully release and hold harmless the Institute, its owners, members, employees, agents, representatives, consultants, independent contractors, volunteers, and others associated with the Institute from any and all claims or liability of whatsoever kind or nature which you might incur as a result of your voluntary decision to participate in any Session as a Practice Client and for authorizing the Program Director(s) to review and evaluate the Session Documentation.
