    Complete the following form to sign up for the 2026 membership year.

  • Partnership Levels

  • PLATINUM LEVEL

    • Placement of your logo and a link to your organization's website on the LVPCA home page
    • Provisions to offer monthly discounts or incentives (up to 12) to LVPCA members through our digital messaging platform
    • Sponsor all four (4) quarterly meetings, including setup of table with materials of your business, and participate in the social meet and greet prior to start of the meeting
    • 1/2 page ad in the LVPCA quarterly newsletter 
    • Twelve (12) social media posts on all LVPCA platforms 
    • Business membership in LVPCA 
    • $750 Annual Fee 

     

    GOLD LEVEL 

    • Placement of your logo and a link to your organization's website on the LVPCA home page
    • Provisions to offer monthly discounts or incentives (up to 6) to LVPCA members through our digital messaging platform
    • Sponsor all three (3) quarterly meetings, including setup of table with materials of your business, and participate in the social meet and greet prior to start of the meeting
    • 1/4 page ad in the LVPCA quarterly newsletter
    • Six (6) social media posts on all LVPCA platforms
    • Business membership in LVPCA
    • $500 Annual Fee

     

    SILVER LEVEL 

    • Placement of your logo and a link to your organization's website on the LVPCA home page
    • Provisions to offer monthly discounts or incentives (up to 4) to LVPCA members through our digital messaging platform
    • Sponsor one (1) quarterly meeting, including setup of table with materials of your business, and participate in the social meet and greet prior to start of the meeting
    • 1/8 page ad (Business Card size) in the LVPCA quarterly newsletter
    • Four (4) social media posts on all LVPCA platforms
    • Business membership in LVPCA
    • $250 Annual Fee

     

    BRONZE LEVEL 

    • Placement of your logo and a link to your organization's website on the LVPCA home page
    • Provisions to offer monthly discounts or incentives (1) to LVPCA members through our digital messaging platform,  business to select the month
    • Sponsor one (1) quarterly meeting, including setup of table with materials of your business, and participate in the social meet and greet prior to start of the meeting
    • A listing in the Friends of LaVista Park section of the LVPCA quarterly newsletter
    • Four (4) social media posts on all LVPCA platforms
    • $100 Annual Fee

  • INSTRUCTIONS FOR PAYING BY ZELLE

    1. Complete the form, enter your signature and hit the submit button.

    2. Go to your Zelle account and add treasurer@lavistapark.org to your Zelle recipients.

    3. Send payment amount via Zelle to the LVP recipient above (treasurer@lavistapark.org), per friends partnership level selected.

      PLATINUM : $750 GOLD : $500
      SILVER : $250 BRONZE : $100

    4. When entering the payment, please indicate the name of your business in the Memo box of the Zelle payment.

    5. Paying via Zelle is a manual process, this form must be submitted prior to going to Zelle for payment.

    2. LVPCA has two options to pay by check: 

        - Drop the check in a secure envelope at the park mailbox located at the park entrance on Brook Forest Drive (preferred)

         OR 

       - Mail your payments to
