By signing and submitting this form, I, the undersigned, hereby agree to the following waiver and limitation and release of liability to Purrista Cat Café, LLC (the “Company”, “Café”):

I waive any and all claims I may have now and in the future, and release from all liability and agree not to sue Purrista Cat Café, Because You Care, Inc., its officers, agents, representatives, volunteers or employees (“Staff”) for any personal injury or loss sustained by me.

I am aware that the Café, although a relaxing environment, may pose certain risks of injury in the course of my time there, including but not limited to: cat scratches, potential allergic reactions, cat bites, COVID-19 exposure, or fleas. I am aware that whenever I come in close contact with a cat, there is a risk that the cat may carry a transmissible disease that may be carried by cats, including but not limited to: roundworms, toxoplasmosis, rabies, ringworm, or Cryptosporidiosis. 

I am also aware that the Café or the Shelter are not responsible for any damage to personal property or stolen goods that may incur while spending time with the cats knowing that these cats come from all different backgrounds including, but not limited to, being homeless, abandoned, abused, neglected and or have health and/or behavioral issues. Therefore, I agree that any damage that occurs or anything stolen is my responsibility such as damage to articles of clothing, purses, shoes and/or personal property.

I acknowledge that the Café receives the cats after they have been vaccinated and medically cleared by the animal shelter. I understand that the Café is not personally responsible for the medical care of the cats.

Further, I agree that I am over the age of 18 and at all times during my visit at the Café, I will abide by and comply with any additional instructions given by the Staff, and I accept that non-compliance may result in injury or termination of my visit. I agree not to handle the cats in a rough manner, nor handle them in any way prohibited by the Company and I will treat them gently and with respect. I will follow all posted rules within the Café regarding acceptable ways to interact with the cats to ensure their safety.

If a staff member or volunteer sees that you are not treating any of our cats or employees with respect, you will be asked to leave.

This Release creates the one and only agreement of the Company and Myself with respect to the subject matter contained herein and sets aside all prior understandings, agreements, representations, and warranties, both written and oral, with respect to such subject matter. If any term of this Release is held invalid, illegal, or unenforceable in any jurisdiction, such invalidity, illegality, or unenforceability shall not affect any other term or provision of this Release or invalidate such term or provision in any other jurisdiction.

I have read this Liability in full before signing below, and I fully understand the contents, meaning, and impact of this Release and agree that this Release will be binding upon my heirs, next of kin, executors, administrators and successors.

IF ANY GUESTS ACCOMPANYING THE ENTRANT ARE MINORS (UNDER 18 YEARS OF AGE), THE PARENT/ GUARDIAN MUST AGREE TO THE FOLLOWING:

I, the parent or guardian of the below listed participant(s), with authority over the care and wellbeing of the below mentioned individuals, will be fully responsible for the juvenile participant, and I am assuming the conditions set forth above for myself as well as the below named individuals. I will be fully responsible for the juvenile participant, and I am assuming and accepting the assumption of risks as described for the following juveniles.
