Drop off / pick up is allowed up to 15 minutes before and after your scheduled camp times. 1/2 payment of any camp will hold your spot, with final payment due by the 1st day of C.A.M.M.P. - or you are welcome to pay in full at any time. Please note: you can be generic or just provide a genre if you want - it's just to familiarize us in advance with the genre / style of music they are interested in and familiar with, individually and as a group, so that we can best create new songs with each group of C.A.M.M.P.ers, on the spot :)
Informed Consent and Acknowledgement I hereby give my approval for my child’s participation in any and all activities prepared by C.A.M.M.P / Creative Sky (led by Jessica Fogle) during the selected camp. In exchange for the acceptance of said child’s candidacy by C.A.M.M.P / Creative Sky / Jessica Fogle, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless C.A.M.M.P / Creative Sky and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected activities. In case of injury to said child, I hereby waive all claims against C.A.M.M.P / Creative Sky including all directors/teachers and affiliates, all participants, sponsoring agencies, advertisers, and owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all performance-related activities, including C.A.M.M.P. Some of these injuries include, but are not limited to, the risk of small injuries or death. While extremely unlikely, this waiver is here as a way to protect our assets. Thank you for understanding.
Acknowledgement of Photo / Video We recognize the need to ensure the welfare and safety of all young people taking part in any activity associated with Creative Sky Performing Arts & Music School.

As your child will be taking part in C.A.M.M.P., we would like to ask for your consent to take photographs/videos of the event or activity that may contain images of your child. These images may be used as a record of the activity or the event, in a written evaluation of the event that will be viewed by future enrolling families, publicity material for further activities or events on fliers/websites, illustrations of the activities or events in published articles, and future grant applications. While our current promotional material is artwork-based and does not use photography or video, for the possible evolution of our brand, promotional materials, and website, we are requesting consent in advance.

We will take all steps to ensure these images and videos are used solely for the purposes they are intended, and please know that no person-specific identifying information will be used (other than date/location of each C.A.M.M.P, and character names). If you become aware that these images are being used inappropriately you should inform us immediately.
