Before you begin

Parents complete a compulsory online course to prepare for baptism, called Belonging (Ascension Press with Fr Mike Schmitz).
This must be completed before booking a baptism.
You need a copy of the certificate (which you will receive after completing the course) to complete this form.

You only need to complete the course once every 4 years; you can use your earlier certificate for a second or third child in that period. If it has been more than 4 years since your last baptism, we ask you to complete the course again.

You also need to attend Sunday Mass with your child the weekend before your baptism to be welcomed before the community.

A brief Baptism Preparation meeting is held regularly to meet you, to answer any questions and walk you through the liturgy.
This meeting must be attended before completing this form.

Baptism
- Baptisms are currently held on the first and third Sundays of the month (only) at St John the Evangelist at 12noon.
- There is no set fee for a baptism. We suggest $50 - $100.
- The weekend before the baptism, we ask you to come to your choice of Sunday Mass, where your child will be welcomed before the community.
- This donation is paid directly to the parish via debit or credit card.

For more information on what the role of Sponsors (or Godparents) is in baptism, read the Baptism Preparation document above. There is room on the certificate for a maximum of up to four Sponsors.
- Only one Catholic Sponsor is necessary.
- Only Catholics can be sponsors.
- Baptised members of other Christian churches may act as Christian witnesses.
- Non-baptised people cannot be sponsors.
The custom in Australia is to have two Sponsors. You can invite others to be there, but please decide the first four (up to) to be officially included on the certificate.
