Bushkids Photo & Video Consent Form

Sign and return the following form if you give photo and video consent to Bushkids.
Bushkids is a program offered through Lahey Consulting. Bushkids often uses photos and video as a means of documenting learning to share with children and families, and promoting nature-based education in the NWT. We want to be able to document our adventures with your child to potentially help other educators offer similar programming, and to record our progress throughout the seasons and the school year.
I am the legal guardian of First Name Last Name and I hereby authorize Bushkids to use photographs and videos of my child and their artifacts (creations) taken during participation in Bushkids programming. This permission includes reproduction of the photographs and videos for any reasonable purpose, in printed, electronic media or social media and sharing with local news media.
The use of photographs and videos may include the following:
- For use on the Bushkids website.
- For use in articles, academic papers, and research relating to Bushkids that may be publishedin local or national journals and books.

All stored versions of these photographs and videos, whether printed or electronic, will be owned by Bushkids. Bushkids will be the holder of the copyright unless agreed otherwise. Personal details, including the photographs, will be disposed of after this specific use.

Bushkids takes your privacy seriously and we have taken steps to protect it. Any personal data you give to us, including photographic images, will be used for the purposes that you have consented to. We will not share your details with third parties without your consent, except where we are legally compelled or obligated to do so. Please note that where you consent to images appearing on the internet, they can be viewed worldwide including countries where Canadian data protection law does not apply.

In giving your consent, you hereby release and hold harmless Bushkids and their owners, operators, agents, employees, representatives and contractors from any and all responsibility or liability for damage of any kind suffered in any manner whatsoever; and you hereby relinquish any and all personal or proprietary rights you may have in connection with such use. You will receive no compensation should any photograph or video of your child be used.
