Affirmation Tattoo Booking

Please fill out the following form to book an affirmation or nipple tattoo with me! My assistant will randomly pick people to receive a scheduling link from received forms.
Affirmation tattoos are $150-250, including the $100 deposit. An affirmation can be any handwritten word or short phrase or tiny doodle - check out my instagram highlight for examples of what folks have gotten! Examples include writing done by the client as well as writing from loved ones. Affirmations may also be computer fonts.

If you will be writing your affirmation out for the appointment (or asking someone else to), I suggest doing so in ball point pen about 0.75 inches - 1.5 inches tall (smaller text is possible but may be more prone to blurring over time). I can also use a photograph of writing, if you have something already written by a loved one. Slightly thicker line weights than ball point pen can be accommodated too. My studio is on the third floor with elevator and stair access. There are shared restrooms available for guests. My table is electric and can lower so you can get on comfortably. My small service dog often is in the studio, but does not need to be if this is an issue for you!
