Classes are held on
Wednesdays, 7:00 - 9:00 pm (Plus and Advanced)
Thursdays, 7:00 - 9:00 pm (Beginners and C1)
at 45 Carlton Street, 2nd floor, September-December 2022.
(2023 schedule will be announced at a later date)

Everyone must be vaccinated at least three times -- three or four doses of a COVID vaccine for extra protection.

Everyone is required to wear a mask that covers their nose, mouth, and chin, without any gaps. Masks and hand sanitizer will be available at the Registration Desk. Please come early to check in.

CODE OF CONDUCT

⚫ I will wear a mask, which covers my nose, mouth, and chin, while indoors
⚫ I am fully vaccinated (three or four doses) plus two weeks;
I will show proof of vaccination at the Registration Desk
⚫ I will answer COVID screening (symptom check) questions upon arrival
⚫ I will wash my hands or use hand sanitizer after each tip
⚫ I will maintain physical distancing as much as possible:
There will be no hugging or physical contact beyond regular dancing
⚫ I will not remove my mask to eat outside food
⚫ I will bring my own water bottle
⚫ I will practise respiratory etiquette (cough or sneeze into my mask or arm)
⚫ I will not invite visitors to attend classes or enter the building
⚫ If I develop COVID-19 symptoms or test positive for COVID-19, I will contact info@trianglesquares.com as soon as possible, so club members can be notified. I will isolate myself for at least two weeks, and ensure that I no longer have COVID symptoms, before returning to classes.

RELEASE OF LIABILITY, WAIVER OF ALL POSSIBLE CLAIMS, AND ASSUMPTION OF RISK

I hereby acknowledge that I have agreed to attend and use the facilities of the Lexington Condominium at 45 Carlton Street, in the city of Toronto in the Province of Ontario, for square dancing classes presented by Triangle Squares (the “Club”).

I acknowledge and accept that there is a risk that I could be exposed to COVID-19 while attending Club events. I also acknowledge and accept that while attending club events, I may, at times, be closer than the recommended social distancing guidelines due to the nature of the activity I am participating in.

I acknowledge and confirm that I am willing to accept this risk as a condition of attending a Club event.

In consideration of the use of the Club facilities, I agree to release the Club (if applicable), their officers, directors, agents and volunteers (the “Releasees”) from any and all causes of action, claims, demands, requests, damages or any recourse whatsoever in respect to any personal injuries or other damages which may occur or arise as a result of exposure to COVID-19 during my visit to the Club event and/or through the provision of services to me by the Club.

I do hereby acknowledge and agree that notwithstanding the generality of the foregoing, I declare that I will not commence litigation or otherwise seek to recover damages or other compensation against the Releasees based on any action, claim, demand, request, loss or any recourse whatsoever arising from any potential or actual exposure to COVID-19 while attending the Club event and/or through the provision of services to me by the Club.

I further acknowledge that the Releasees can rely on this Release of Liability, Waiver of all Possible Claims and Assumption of Risk as a complete defense to any and all claims, damages, causes of action, or recourse or liability that may arise at any time.

I have carefully reviewed this Release of Liability, Waiver of all Possible Claims and Assumption of Risk and acknowledge that I fully understand the terms as set out above. I acknowledge that I am signing this Release of Liability, Waiver of all Possible Claims and Assumption of Risk voluntarily.
