1. I understand payment for MexiVet Express services including veterinarian fees paid by MexiVet Express on my behalf) are my responsibility, in full, and must be paid before the pet leaves the veterinary clinic.
2. I understand that the pricing quote MexiVet Express provided prior to the in-person consultation at the clinic is subject to change depending on the physical assessment provided by the veterinarian.
3. I agree that I will provide MexiVet with a valid credit card to be kept on file in the Time to Pet Portal. I authorize MexiVet Express to charge my card on file for any outstanding balance the day after my service. I agree that if I have any issues with the vet charges that MexiVet incurs on my behalf, I will notify MexiVet upon receipt of my invoice on the day of service.
4. I agree that invoice payment is due upon receipt. Be advised that we will charge 2% interest per month on late invoices.
5. I understand that if I fail to pay the total amount due for the Services within 30 days of the date of the invoice, MexiVet Express may refer the matter to a collection agency. In such an event, I will be responsible for all costs and expenses, including reasonable attorney’s fees, incurred by MexiVet Express in connection with the collection of such unpaid amounts. MexiVet Express reserves the right to terminate this Agreement and to pursue all available legal remedies.
6. I understand that I assume responsibility for all risks associated with the transportation of my pet.
7. I understand there are risks involved with transporting a pet such as, but not limited to, inclement weather, other drivers, problems with the road, animals on the road, problems with the car, problems with the restraints of pets within the car, etc. I do hereby waive and release MexiVet Express, its employees, and affiliates, from responsibility, including financial, for the above stated circumstances and any other circumstances that can cause the loss of control of the vehicle resulting in death or injury to my pet(s).
8. I understand that I assume all responsibility, including financial, for any medical costs that may arise in the event my pet(s) bites, attacks, or in any way harms MexiVet Express, its employees, or affiliates. I assume all responsibility, including financial, for any property damage caused by my pet(s) while in the care of MexiVet Express, its employees, and affiliates.
9. I undertand that MexiVet Express is a transportation service and does not provide veterinary care, nor does it have any influence over, or control of, decisions made by any veterinary clinic with which it associates regarding care, treatment, recommendations, or medical opinions.
10. I understand that MexiVet Express is not responsible for medical outcomes - it only facilitates transportation and communication between veterinary professionals and pet owners. I agree not to hold MexiVet Express liable for results of services rendered by the veterinarian or their staff.
11. I understand that MexiVet does not endorse or guarantee any veterinary costs, quotes, or prices and is not responsible for the costs set by the veterinary clinics. You agree not to hold MexiVet Express liable for any discrepancies or disputes that may arise from the estimates or prices of other companies.
12. I understand anesthesia and medical treatments involve some risk to my pets I will not hold MexiVet Express, its employees, and affiliates responsible for death or complications that may arise as a result of anesthesia or treatment received at the veterinary clinic.
13. I understand that my pet must be fully fasting from food and water for a minimum of 10 hours, if they are to undergo anesthesia and/or receive blood work. Certain medications are allowed on the day of appointment on a case-by-case basis, please contact us directly.
14. I understand that MexiVet Express is not responsible for any personal items that may be lost during my pet’s appointment. Examples of personal items include but are not limited to collars, leashes, harnesses, bedding, food and toys.
15. I understand that if my pet has aggressive tendencies, it is my responsibility to securely place a muzzle on my pet prior to their departure. If the muzzle comes off during my pet’s transport and the driver or clinic staff cannot safely remove my pet from the vehicle, then my pet will have to return to my residence without having received veterinary services and I will still be responsible for the transportation cost.
16. I understand that the MexiVet Express driver will send their estimated time of arrival in advance and that I should be ready by their arrival time. Drivers will wait a maximum of five minutes, any minute past five minutes will be charged at a rate of $1 per minute.
17. MexiVet Express can use images of my pet, and a summary of their treatment and costs in their marketing.
