INFORMED CONSENT RELEASE
I fully understand all the questions above and have answered them all correctly and honestly. I understand that the services offered are not a substitute for medical care. I understand that the skin care professional will completely inform me of what to expect in the course of treatment and will recommend adjustments to my regimen if deemed necessary. I also am aware that individual results are dependent upon my age, skin condition, and lifestyle. I agree to actively participate in following appointment schedules and home care procedures to the best of my ability, so that I may obtain maximum effectiveness.
In the event that I may have additional questions or concerns regarding my treatment or suggested home product routine, I will inform my skin care professional immediately. I release and hold harmless the skin care professional Danielle Darmetta of Boutique Beauty harmless from any liability for adverse reactions that may result from this treatment.
CANCELLATION & LATE ARRIVAL POLICY: Your appointment has been reserved exclusively for you. If you need to reschedule or cancel, please text me 561-544-7002. A 48 Hour notice is ideal in order to be able to fill the spot held for you. No shows or cancelling with less than 24 hours notice, will result in a charge equal to 25% of reserved appointment(s)
You will receive a courtesy, text and/or email to confirm the date and time at least 72 hours prior to your appointment. If we are unable to reach you and only leave a message, or cannot reach you at all, please understand that it is your responsibility to remember your appointment dates and times in order to avoid missed appointments and cancellation fees. You are always welcome to call or check your confirmation text/email to double check any appointments if you’re unsure.
Late Arrival We will always try our best to accommodate you if you’re running behind, stuck in traffic, etc. It happens, we know! However, your tardiness can affect the remainder of the day by delaying clients who come in on time. For this reason, we have set a few general ground rules for such situations. Clients will generally be allowed a 10-minute grace period. After that time, we will call to check in on you. If you are able to make it in time for your entire service to be completed, great! If not, you may have to forgo parts of the service in order to keep it in the time allotted for you.
