    If this form is not received/submitted by the due date/time, you will not be allowed to show Horse & Pony at the 2026 Dane County Fair. You will receive an email confirmation at the provided email address upon successfully submitting this form. Submit this form only once per Exhibitor, thank you.

    Each Horse & Pony Exhibitor, including Horseless Horse members, must submit this form for their project animal(s) they intend to show and agree to the Horse & Pony Exhibitor Behavior Policy (below).  It is not required to declare two horses if you only intend on showing one horse. A maximum of two equine may be shown by an Exhibitor.

    **If you have already completed your declaration form and would like to change it, please re-submit this form.**

    Animal Substitution Policy:
    If the declared animal dies or becomes lame or injured, you may request a substitution as follows:
    a) Submit a written request prior to July 10, 2026 with a letter from your veterinarian as to the reason the animal will not be able to attend.
    b) Send request to Horse and Pony Dept. 6, Dane County Fair, via e-mail to info@danecountyfair.com.
    c) All substitutions are evaluated on an individual basis and are subject to the superintendent’s approval.
