Thank you for considering Leviticus for your next tattoo. This form is the first step in our collaborative process. Fill it out in as much detail as you can. The more we know, the more efficiently we can bring your vision to life. We read every submission and appreciate your patience as we give each idea the attention it deserves.
To ensure we create the best possible artwork, our artists focus on the styles and imagery they are most passionate about. While your idea may be great, we might not be the perfect fit. In that case, you will still receive a response. Our goal is to ensure you get the best version of your tattoo, even if it means recommending a different artist or studio.
In the pursit of making tatttoos that look great 10 years from now, we do not tattoo the side of fingers. Per artist preference and comfortabililty we do not tattoo genital areas. Tattoos on the hands, neck, face, and ears are considered on a case-by-case basis.
Once the initial consultation process is complete, we will provide a booking link so you can find a time that works for you and your artist. We may start with an in person consultation to discuss additional details or book straight into your appointment depending on your idea.
We require a $100-$250 non-refundable deposit to book any tattoo appointment. Deposits for in person consulations are refundable and range from $50-100.
We are unable to book an appointment for you without a completed consultation form and a deposit.
