Public Health encourages schools and families to verify student’s COVID vaccination status as part of our effort to keep more students in school safely. Washington Department of Health (DOH) requires that students who are COVID positive to isolate at home, and those who are close contacts to quarantine. The duration of the quarantine period is determined by vaccine status.
COVID vaccines are NOT required to attend school at this time, but this information does help our nurses respond effectively when contact tracing.
Acceptable forms of vaccine documentation:
- Photo or copy of official vaccination card
- Documentation from a health care provider (doctor, clinic, etc.)
- Print out from State of WA Vaccination Record System
- WA Verify - Digital COVID-19 Verification Record
Vaccine documentation must show the name of the person receiving the vaccination, vaccination dates, and vaccine type.
If you have already submitted vaccine data, there is no need to submit it again; but if your student has recently received a booster or you do not remember whether you have entered data or not, please resubmit.
