Program Rules:
I/we, Full name,acknowledge that the Transcend STEM Education (herein referred to as the agency) and I/us (herein referred to as client and/or participant) have received these policies and procedures and they have been explained to me/us.
 
·      I/we understand that program participant has a 10 day window to review course content and decide whether to continue or drop out of the course by contacting program staff.
·  Should participant enroll and fail to attend(and fail to notify program staff within the 10 day window) - unless there are extenuating circumstances participant will be required to reimburse any grant funding used to pay for their course.
·      That once enrolled, I/we must adhere to program rules and any violation of program rules may result in I/us being removed from the program immediately.
·      ALL issues, anything pertaining to your program while in the program should be brought to the attention of program staff immediately.
I/we further attest that;
o  No drug use or other illegal activities will be engaged by participant while enrolled in the program. (PLEASE NOTE: Transcend STEM Education and its affiliates has a ZERO TOLERANCE on Drug use/sale or other illegal activities, and YOU WILL BE REMOVED FROM PROGRAM IMMEDIATELY WITHOUT NOTICE.
I/we furthermore acknowledge that
o  Funding is on first come first serve NOT guaranteed and is based on client eligibility and availability of funds.
o  While the agency may assist with program funding, that the agency is not obligated to pay for my/our program stay –it's all dependent on eligibility, program adherence and availability of funds.
o  That I/we (the participant) will be required to pay ALL or a portion of our expenses (only where applicable based on program agreement) .
o  That I/we need to be able to have an exit strategy from program and that Transcend STEM Education is only providing training for the duration agreed upon.
o  That no guarantees or promises have been given me/us as part of the program.
o  That if any changes in my/our situation occur which would potential impact my/our ability to stay in the program, I/we will notify the appropriate program manager promptly.
Privacy Policy:
Transcend STEM Education collects client data and documentation for the following purposes:
• To provide the most effective services, we need an accurate count of all people included and potentially receiving impact from the services whether directly or indirectly.
In order to ensure that clients are not counted twice, we need to collect four pieces of personally identifying information. Specifically, we collect name, birth date, and race/ethnicity. You may also choose to provide your social security number. However, signing this form does not require you to do so. Your information will be stored in our database.
 • We use strict security policies designed to protect your privacy. Our computer system is highly secure and uses up-to-date protection features such as data encryption, passwords, and two-factor authentication required for each system user. There is a small risk of a security breach, and someone might obtain and use your information inappropriately.
• Only a limited number of staff members, who have signed confidentiality agreements, will be able to see this information.
Client Termination & Grievance Policy:
Should participant violate program requirements (e.g., fails to advise of intent to withdraw during the grace period) or no longer meets minimum eligibility requirements for assistance (e.g., income change), violates our zero tolerance to drugs policy, we may terminate assistance.
To terminate assistance, we will either:
·      send written notice to the participant containing a clear statement of the reasons for termination, OR
·      call participant and advise them of our decision to terminate OR
·      send a letter in the mail to the participant containing a clear statement of the reasons for termination.
Participant will have the opportunity to:
Review the decision,and an opportunity to present written or oral objections before a program staff member other than the program staff involved in the disagreement(or a subordinate of that person) who made or approved the termination decision.
Please note: Termination does not bar Transcend STEM Education from providing further assistance at a later date to the same family or individual. Transcend STEM makes available to participants their rights and obligations regarding their participation in the project.
By signing below, I/we acknowledge that we have had the program requirements explained, read, and understood the above as well as all other program documentation pertaining to the Transcend STEM Education program being applied for.
 
Hold Harmless/Indemnity: I/we acknowledge that I/we will agree to indemnify and hold harmless Transcend STEM Education for and from any claims, causes of action, or any other proceeding of any type or kind that are made against Transcend STEM Education (and/or its affiliates) where such claim, cause of action or other proceedings arise from the conduct, act, omission, or commission by the other party.
