    The application process has been designed to help bidders develop their ideas and show how the suggested project will reach the outcomes of the project.

    To apply you must:

    • Complete each section in this form. You must write in the text box for each section. The minimum font size is Arial 11 and you must keep within the word count provided. If you write more the additional information will not be read. This is to make sure that everyone making an application is treated fairly in how much information they can write.
    • Complete the declaration.

    At this stage you are not being asked to demonstrate your organisation’s financial or legal status; this is to avoid unnecessary work for applicants.

    Each part of the form must be filled in before being submitted. If there are any gaps the application won’t be considered. All applications will be considered by a panel of people who have relevant knowledge, interest and / or experience. All applications will be assessed against pre-agreed criteria and information about the criteria and scoring is included below.

  • Assessment Criteria and Scoring

    LOPF is looking to fund activity that will contribute to the outcomes of the overall programme. Each member of the assessment panel will assess each application received individually. The panel will include:

    • A representative from Leeds Community Healthcare
    • An older person with lived experience
    • A member of LOPF staff
    • A representative from Leeds City Council

    Each application will be assessed and scored against the same criteria. A maximum of 100 points can be scored for the quality of the application. The answers to each section will be scored using a range of 0 – 20, 20 being the highest possible score and 0 being the lowest possible score.

    Applicants must score a minimum of 60% of the overall quality points available (i.e. 60 points out of 100). The successful applicant(s) will be required to sign a delivery contract with Leeds Older People’s Forum before delivery can commence.

    Leeds Older People’s Forum, as the lead partner for this project, reserves the right not to award any contract if they are not satisfied with the quality of the applications received.

    • The target group for this fund are people who have received or are receiving support from LCH’s Neighbourhood team, they may be restricted to their home environment and as a result need additional support. We believe that this project should give partners the extra resources needed to receive referrals from health and social care and as a consequence provide timely person-centred support.


      The strongest answers will be those drawn from your experiences of working with individuals and will be backed up with evidence. Evidence could include some of the following: feedback from older people, consultation, statistics, quotes, information gathered from pilot or existing projects that you run or information about referrals that you cannot currently meet.

    • This project has five outcomes:

      • Empower individuals to improve their social connections, health, quality of life and/or wellbeing to prevent admission to, or following discharge from, hospital
      • Take a person-centred approach by coproducing flexible, effective and tailored cross-sector support to identify goals and improve outcomes for individuals
      • Enhance the NHS by investing in third sector services to complement clinical service provision and reduce delays in hospital discharges
      • Enhance partnerships between third sector organisations, health professionals and individuals to improve efficiency, share up-to-date information, resolve challenges and evaluate impact
      • Use a Test and Learn approach to build on our understanding of ‘what works’ in Leeds
