As a not-for-profit landlord, City on a Hill must submit a bi-annual report to the City of Milwaukee documenting the charitable uses of the facility by our own organization and all tenants. Please upload to this application any and all of the following documents:

- General program materials (i.e. brochures regarding services provided)
- Certification of Liability Insurance
