Tattoo Request Form

Thank you for enquiring about getting a tattoo with Jasmin Walsh. We’ve created this form to collate as much information as possible in order to collaborate together and create a beautiful artwork for your new tattoo. The more information Jasmin can get beforehand, the more time we save in the long run. 

Jasmin prioritises the requests that align with her style and portfolio, so please ensure you’re familiar with her work before going ahead. Fine line, dot work, elegant tattoos are chosen over heavy, more “old school” style. 

Part of Jasmin’s design process is to sit with you, the client, on the day of your booking and draw your new piece with you present, so you can input exactly what you want into the artwork. The time for this is included in the appointment you are offered. For larger pieces we can offer video consultations to collaborate on a piece, but we will let you know all these specifics when you book in. This is an unusual approach, but presents an authentic and truly collaborative approach to tattooing. 

Because of this, Jasmin does not make direct copies of other artist’s work. However if there is a theme or motif you like, you can offer that and we can merge ideas and images to create something unique to you.

 

Whilst Jasmin does cover up old tattoos, please undertand that your ideas will need to be flexible and Jasmin may request an in studio consultation in London or via video call. Not all cover up requests will be undertaken. 

 

All tattoos will take place at the Oh So Fine Tattoo Studio - 3rd Floor 309 Upper Street London N1 2TU

Thank you for providing us with all this information. We will respond to you as soon as we can with a quote and an appointment. Whilst we endeavour to respond with a quote within 24hrs, please be aware that this is not always possible, and a response may take up to 72 hours. 

We require a deposit to hold your appointment, which will then be taken off the total amount paid on the day. 

Deposit:

£100 Session - per tattoo

£250 Day Session.

 

Deposits are non refundable.

 

Any no-shows or failure to contact Jasmin to rearrange the appointment within 48 hrs of the scheduled appointment will result in a loss of the deposit. Any cancellations will require a new deposit.

Your quote is valid for 30 days from the day you receive it. If you go over these 30 days then please fill out the form again.

The tattoo quote is an estimate of how long it will take Jasmin to complete your piece depending on size, location and design. If your project alters in any way on the day of your appointment, this can impact on the final price for the tattoo. You will be advised on any changes of price prior to your tattoo. 

A submission of this form is an agreement to these terms.
