PHOTO / VIDEO RELEASE 

 

I hereby grant to Fonda Interactive Training (F.I.T and to its employees and agents and assigns, the right to use my likeness, image, photograph, or video in any and all of its publications, including websites, with or without payment or other additional consideration. I understand and agree that any such materials are the property of Fonda Interactive Training (F.I.T and will not be returned.

I hereby irrevocably authorize Fonda Interactive Training (F.I.T to edit, alter, copy, exhibit, publish, or distribute such likeness, image, photograph, or video for purposes of publicizing Fonda Interactive Training (F.I.T, its programs, services, products and for any other lawful purpose. In addition, I waive the right to inspect or approve the finished product, including but not limited to any written or electronic copy thereof wherein my likeness appears. Additionally, I waive and disclaim any right to royalties or other compensation arising from or related to the use of any such image, likeness, photograph, or video of me.

I hereby hold harmless and release and forever discharge Fonda Interactive Training (F.I.T from all claims, demands and causes of action which I, my heirs, assigns, representatives, executors, administrators or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

I certify that I am 18 years old or older, and am competent to enter into this agreement. I have read and reviewed this release before signing, and my signature below indicates that I fully understand the contents, meaning and effect of this release.
