AUTHORIZATION FOR IBEW DUES DEDUCTION
I hereby authorize my employer JEA, to withhold from my paycheck the number of dues required by IBEW Local 2358, as stated in their by-laws and transmit it to IBEW Local 2358 bank account. The IBEW 2358 dues at the present time are which will be deducted bi-weekly from my salary. I understand the dues may change or fluctuate depending on the IBEW Local 2358 by-laws, and authorize my deduction to fluctuate accordingly.

I understand that I may terminate this authorization by notifying JEA’s payroll department.
