CASE SUBMISSION REQUIREMENTS:
1. Use of the provided PowerPoint template is required
2. Presentations cannot exceed 10 minutes in length
3. Cases must have a known diagnosis. This diagnosis is not shared with the audience until the end of the presentation.

SLIDE 2: Title of the case (do not use the diagnosis in the title of the case), your full name and name of institution, city, state and country

SLIDE 5: Imbedded video cannot exceed 30-45 seconds in duration
SLIDE 6: Audience discussion: Phenomenology, Differential diagnosis, Investigations
