Any and all events, competitions and activities conducted by BTI Events LLC (“BTIE”) and United States International University Sports Federation ("USIUSF") for the World University Games, World University Championships, or other university competition sponsored by FISU (“Event”), or in which I am representing USA University Team, (“USA Team”).

General Acknowledgement & Acceptance

I hereby acknowledge that all information and signatures on this form are applicable to my participation in the Event.

Oath of Eligibility

I declare that I am eligible and in good standing with regulations laid down by USA Team, the International Federation for University Sports (FISU), and the United States International University Sports Federation (USIUSF. I also declare I am not under suspension or disciplinary action imposed for use of illegal drugs or other athlete regulation infractions. I agree to sign additional documents to this effect when required to do so by USA Team, the USIUSF, or the local organizing committee.

I certify that, to the best of my knowledge and belief, I am in good physical condition and have no disease or injury that would impair my performance in this activity and agree that while a participant in this activity, I will keep myself in top physical condition and retain my eligibility status.

I agree to notify USA Team immediately upon any change to my eligibility, any disciplinary action, or any change in my physical condition that would result in the declarations and certifications becoming untrue with respect to any Event.

Each ATHLETE must meet the following criteria in order to be eligible.

1.    United States Citizen and able to obtain a US Passport.  Passport’s expiration date is at least six months after the conclusion of the Event.

2.    Be at least 18 and no older than 25 years of age on 31 December of the year of the event.

3.    Have taken at least one 3-credit course towards a degree or diploma in 2024 or 2025. Incoming freshmen and transfers are usually eligible. Progress towards a degree must be able to be demonstrated.

4.    Submit the Certificate of Academic Eligibility Form signed by your university or submit the form with Section 1 filled out and send a current transcript or a Current Enrollment Verification Certificate with your legal name and the current academic year on it. RETURN ORIGINAL TO BTIE by electronic submission. The form states that the athlete is PROGRESSING towards graduation or has graduated in the last year.  Students in Masters or Doctorate programs are eligible.

5.    Sign the “USA University Team Honor Code and Code of Conduct” form and sign the “USA University Team Authorizations, Waiver and Release Form” and send to BTIE.

Each OFFICIAL and STAFF person must meet the following criteria in order to be eligible.

1.    Sign the “USA Team Honor Code and Code of Conduct” form and sign the “USA Team Authorizations, Waiver and Release Form”. USA Team and all participants agree to adhere to all NCAA (or NAIA, NJCAA), USIUSF, and FISU regulations necessary in participating in a GAMES.

Doping

I understand that it is the duty of individual members of USA Team, including athletes, athlete support personnel, and other persons to comply with all anti-doping rules of the World Anti-Doping Agency (“WADA”), including the U.S. Anti-Doping Agency (“USADA”) and all other policies and rules adopted by WADA, USADA, NCAA, NAIA NJCAA, and FISU.

I understand that it is the policy of USA Team to discourage and prevent the use of prohibited doping substances and prohibited doping methods. I know that I can call the USADA Drug Reference Line (719-785-2000) or check at the Drug Reference Online (www.globaldro.com) to check the status of all medications and substances I am currently using, and to satisfy any questions or concerns that I may have about medications and prohibited substances and methods.

I acknowledge that the use of prohibited substances or prohibited methods may subject me to disqualification and other sanctions imposed by USADA, WADA, NCAA, NAIA, NJCAA, or FISU.

Drug Testing

I understand that testing organizations (USADA, WADA, etc.) may conduct drug testing on athletes during and outside of competition. By participating in the Event, I hereby consent to and authorize the testing organizations (USADA, WADA, etc.) to test me for prohibited substances and prohibited methods at any time. I further acknowledge and agree that my refusal to participate in a drug test may subject me to disqualification and sanctions, the same as if I had tested positive for a prohibited substance.

[CB1] In the event I refuse to submit to a drug test or a drug test has a positive result, I recognize that I am subject to the adjudication processes established by the US Anti-Doping Agency (USADA) or other appropriate agency, [CB2] because of my refusal or positive test.

Random drug testing may be done on the athletes.  All athletes that win a medal will be tested.  The tests are done by WADA[CB3]  and positive results can affect NCAA, NAIA, or NJCAA eligibility.[CB4] 

Authorization & Event Promotion

I agree to be filmed, audio recorded, and photographed by the official photographer(s) and network(s) of USA Team under the conditions authorized by USA Team and give the Event organizers and USA Team the right to use my name, picture, likeness, and biographical information before, during and after the period of my participation in these activities to promote the activity in which I participate or to promote the success of the team on which I compete.

I understand and agree not to use or authorize use of pictures of myself in the uniforms and equipment provided by USA Team for the purpose of trade, without the consent of USA Team.

Authorization for Medical Services

I hereby give consent for USA Team to provide or obtain for me medical care and treatment and emergency medical services for any injuries arising in any manner from my participation in the Event. Additionally, I hereby agree that, in the event I elect to obtain any of these services or treatments from any sources other than that provided or obtained by USA Team, I shall accept full and complete responsibility for any consequences of that decision.

I further authorize release of any medical information necessary to process a claim for accident/medical payment insurance for an injury or illness incurred while I am participating as a member of the USA Team delegation in the Event.

Risk of Serious Injury

I understand and appreciate that my participation in the Event and in my sport carries a risk of serious injury, including permanent paralysis or death. I voluntarily and knowingly recognize, accept and assume this risk.

 

WAIVER & RELEASE

In consideration of allowing me to participate in this activity, I hereby release and hold harmless USA Team, BTI Events LLC, and USIUSF, members of its board of directors, and its officers, employees, members, volunteers, other participants, and agents (collectively, the “Released Parties”), of and from, and do discharge and waive, any and all claims, demands, losses, damages, and liabilities that I may have or sustain with respect to any and all damage and/or injury, of any type, arising out of my participation in the Event and any USA Team activity, except as may result from the sole negligence or willful misconduct of USA Team, BTIE, and the USIUSF.
