This form is being submitted after 09/11/26 and is subject to the $20 Entry Fee, plus $100 per entry late fee (Max. $500)

This form will be disabled two days prior to the audition date. Any entries after this form has been disabled will need to be made on the On-Site Emergency Entry Form.

High School Jazz Choir Auditions will be held on Wednesday, October 07th at SNU in Bethany, OK. High School Jazz Choir only has one audition!!

The 1st round auditions for JH All-State will be held Saturday, October 31, 2025, in your geographic region (Quad). Quad assignments are the same as OkMEA. A list of Quad assignments can be found on the OkCDA or OkMEA websites. 2nd round auditions for qualifying junior high voices will be on Tuesday, November 17, 2026, at Rose State College, 6420 SE 15th St., Midwest City, OK 73110

STUDENT ENTRIES (Voice Parts should be entered with an upper case letter followed by a 1 or 2. (Soprano 2 would be enter as S2). Please note Jazz Entries have no 1 or 2 only SATB) Make sure you click on Save and Add after entering the last student's information.
OkCDA Student Entry Invoice YOU MUST ENTER THE TOTAL NUMBER OF STUDENTS YOU'VE SUBMITTED. The fee is $20 per entry, plus $100 per late entry (Max $500). If you have more than five students listed on this form you will receive an updated invoice from the Executive Director which will show the $500 maximum.

The fee includes $20 per student plus $100 late fee per entry (Max $500). If you have more than 5 students your invoice price on this form will be incorrect. A corrected invoice will be sent by the OkCDA Executive Director.

School checks and money orders are the only payment methods accepted. School checks replacing Purchase orders for audition entries and/or festival participants' registration should be paid prior the January OCDA Convention/Honor Choir Festival. Purchase orders must be paid by April 1, 2027, in order for students from that school to participate in subsequent festivals.

Credit Card Payments are also accepted. You can find the Donation/Payment link on the bottom of the first page on the OkCDA App or on the website.

   I recommend the students I've submitted as persons/performers suitable for membership in the OCDA All-State Choir. I understand that my students may audition only if I am present at the audition site and a current member of ACDA. I also certify that a student will not audition if academically ineligible according to OSSAA rules. I understand that I am required to work at both audition sites for the entire day and will make arrangements for my students to be transported and supervised to and from the audition sites.
