2022 Summer Camp Registration Form

Instructions:

1. Fill out one form per player.

2. ORC follows the US Squash COVID protocols. All unvaccinated players are required to wear masks.

3. All players must have a waiver and emergency contact form on file. If you do not have both of these forms on file, please fill them out by clicking on the links above.

4. Member accounts will be billed on June 1. Non-members will be billed immediately, but all refund requests will be honored through June 1 (net of a 3% credit card processing fee).
