本人願意遵守上課時各事項如下：
1) 確保出席全部課程，每次上課不會遲到、早退或缺席。否則會被取消上課資格。
2) 上課第一天到會場登記時，要帶備報名回條，以作登記及領取學員上課證之用。
3) 上課第一天必須最少一小時前及之後五天最少半小時前, 到達會場, 排隊登記, 依次序入座。學員如要與某學員同座, 一定要一同排隊。
4) 非學員及幼童不得入場。
5) 上課期間將手提電話或其他發聲器材的聲音關閉。
6) 上課期間不准許拍照、錄影 或 錄音。
7) 不可在課堂場地內進食。學員可自備清水飮用及所需筆記用品。
8) 如在上課期間有任何查詢或特別事故，可與學員服務處接洽。
9) 如需借用傳譯機，第一天報到時，可到傳譯機服務處繳交$50按金。此按金於交還傳譯機時退回。
10) 六天的上課期間内，應停止抽煙、喝酒及性行為，以得到最佳開穴效果。

學員守則
1) 到會場報到及上課時要保持安靜及遵守大會規則，並聽從各義工的指示。若行為不當，會被邀請離場。
2) 不可在上課期間或所有與多倫多長生學協會有關的活動內進行銷售、推廣、政治或宗教等活動。
3) 不可向長生學調整病患者收取任何利益或費用。
4) 未經多倫多長生學協會的許可，不准以長生學的名義在外設立長生學調整站或以長生學的名義進行與長生學有關或無關的籌款活動。
5) 不得將長生學課本或上課的筆記和內容借與沒有上過長生學之人士分享。
6) 不可將其他學員或調整病患者的聯絡或有關個人資料外泄。
本人承認主辦單位對學員在上課及非上課時間，不負任何事故的法律責任。學員簽字同意放棄一切法律訴訟主辦人及主辦單位的權利。本人如有違反以上規則，願意接受長生學協會的處分，包括除去長生學學員的資格及退還長生學所有課本材料和證件。
本人清楚明白以上加拿大多倫多長生學協會學員協議書的各項內容，自願簽署承諾。
Terms of Longevitology Course Attendance
1. Students must attend every class of the course and must not be late or leave early without proper reason. Otherwise, students will not be allowed to continue with the course.
2. On the first day of class, students must bring the receipt provided to them during application submission for registration purpose.
3. Students should arrive at least 1 hour before class starts on the first day and at least half an hour early on the subsequent 5 days to line up for registration and be seated sequentially under guidance of the onsite volunteers. Students who wish to be seated together must line up together.
4. No non-registered persons and children will be permitted into class.
5. All mobile and audio devices must have the sound switched off during class.
6. No photographing and audio/video recording is permitted during class.
7. No food is allowed in class. Students can bring in drinking water for consumption and other necessary stationeries.
8. Student Service counter is there to handle any student queries and special circumstances during class.
9. Students who require a listening device for interpretation service, must provide a deposit of $50 at the Interpretation Device counter. This amount will be refunded when the device is returned.
10. Students should refrain from smoking, alcohol or sexual activities during the course, in order to receive maximum benefit from chakra opening.
11. If a student is unable to attend the course before it starts, Mr. Paul Lai must be immediately notified at 416-930-9128 or at longevitology.tor@gmail.com,
Code of Conduct for all Students and Graduates
1. I will remain quiet and orderly during registration and class attendance and I will follow the instructions of the volunteers on site. I understand that I will be asked to leave for any disruptive behavior.
2. I will not conduct any personal business during class or any Longevitology Toronto activities, including but not limited to, any activities of sales, promotion, religious and political nature.
3. I will not accept any monetary or non-monetary gifts for Longevitology adjustment service to others.
4. I will not use the name of Longevitology to conduct any activities that are not approved by Longevitology Benevolent Foundation in Taiwan and Longevitology Toronto Canada. This includes but not limited to, forming Longevitology groups, training others to perform Longevitology adjustment, opening Longevitology adjustment centers and fund raising.
5. I will not share the Longevitology textbook or any notes and information from the course with any persons who have not attended the Longevitology course.
6. I will not disclose to others, any contact and personal information of other students and adjustment patients. I understand that if I violate any terms and condition in the Longevitology Toronto Canada Student Agreement, Longevitology Toronto Canada has the right to terminate my attendance of the course or cancel my membership at any time and demand the return of the Student ID card and course material.
I hereby agree to assume full and complete responsibility for my personal safety and personal property in connection with any Longevitology Toronto Canada course and activities that I participate in. I hereby waive any right to bring legal action and claim against Longevitology Toronto Canada and its management at any time. This release is for any and all liability for personal injury, property loss or damage in connection to any Longevitology Toronto Canada activities.
I fully understand the above statements and will abide by all the terms and conditions set forth in this Longevitology Toronto Canada Student Agreement.
