Appointment Cancellation Policy
In pursuit of our commitment to delivering exceptional health care, we ardently serve every patient with the highest standards of promptness and quality. We kindly bring to your attention that unattended appointments, tardiness, and last-minute cancellations not only disrupt the schedules of our practice but also affect the timely service to our other esteemed patients. We respectfully request your cognizance and adherence to our established policy concerning appointment attendance.
Appointment Cancellation
When you secure an appointment with us, you reserve a specific time slot which consequently becomes unavailable to other patients. In the spirit of mutual respect and consideration for others, we kindly request that you promptly notify our office at (773 365 1300) — either by call or text — should you ascertain that you will be unable to attend your scheduled appointment.
For necessary cancellations, we stipulate a minimum notice of 24 hours. Given the high demand for appointment slots, your timely notification will grant another patient the opportunity to avail themselves of that particular time.
Late Cancellations/No-Shows
Cancellations made less than 24 hours prior to the scheduled appointment time are deemed as 'late cancellations' Failing to attend an appointment without any prior cancellation notice is classified as a 'no-show'
We wish to emphasize that when you secure an appointment with us, a dedicated time slot in our calendar is exclusively reserved for you. As such, should you either cancel with less than 24 hours' notice or fail to attend, a rebooking fee of $75 will be levied.
To avoid incurring this fee, we kindly urge you to provide a minimum of 24 hours' notice should you need to cancel or modify your appointment.
