    First Aid and Medical Treatment
  • NOTE: Ofsted require us to request the permission of Parents / Carers for Emergency Medical Treatment. However, the medical profession take the view that the parent's or carer's consent to medical treatment cannot be delegated. This view is explicit in the Children's Act 1989. Thus, medical consent forms have no legal status and a doctor or nurse insisting on the consent of a parent / carer has the right to do so. For this reason we cannot insist on parents giving consent by signing the statement above. However, it can be a comfort to medical staff to have general consent in advance from parents / carers or to have a member of staff on hand able to sign forms required by medical authorities if parents / carers cannot be contacted.

  • Suncream

  • On days where the UV is high it is important that you send your child into preschool with suncream on. We will then reapply it throughout the day.
    At preschool we use Aldi's Lacura factor 50 children's suncream. If your child has allergies or you would prefer them to wear a different suncream please supply preschool with a named bottle that we can keep at the setting.

  • Observations

  • In accordance with Department for Education recommendations, we are required to keep records of our pre-school children’s progress. This procedure involves observations made by each child’s key-worker (a member of staff with a special responsibility for a group of children) or the supervisor. Our registration on the Early Years Register means that this requirement extends to all children in our care, including those attending out of hours provision who are older than the early years age group.

  • Intimate care

  • We are required to obtain permission from the parents/carers to assist the children with their intimate care in accordance with our policy. All of our staff are experienced in assisting children with their intimate care. They undertake this with sensitivity and respect for the dignity of each child.

    We will work with our parents/carers to assist and enable the children to manage their own intimate care as soon as they are able to do so. There may be medical reasons why this is difficult for some children and we would be able to draw up a health care plan for children in these circumstances in full consultation with parents and any other health care providers.


    We would be grateful if you could complete the section below giving consent for our staff to carry out intimate care procedures when necessary.

    • I give permission for preschool to provide intimate care to my child
    • I will advise the school of anything that may affect issues of personal care (if medication is changed or my child has an infection for example)
    • I understand the procedures that will be carried out and will contact the preschool immediately if there are any concerns.
  • Photo Consent

  • As part of our Preschool activities we may take photos of the children, the photos may be used in the following ways:

    To be used for display purposes around the setting

    To be used on our Preschool website

    To be used on our Facebook Page (the children’s faces will not be covered)

    To be used for publicity material such as the Preschool brochure

    We need your permission to be able to take photographs of your child. Please confirm below your consent preferences. You can withdraw your consent at any time, if you wish to withdraw your consent, please do so in writing.

    Please note, this is in addition to Photos taken and uploaded through the secure online progress tracking tool Famly, where consent is obtained separately.

  • Please note that signing above does not constitute an application for a place at Primary School. Applications for Primary School places can only be made by parents directly to the Devon County Council School admissions team; we are not party to the admissions process. However, we usually receive publicity information about admissions during the application window (typically 15th November to 15th January prior to the September when your child is due to start school), so please check our notice boards during this time.

  • SHARING INFORMATION WITHOUT CONSENT

  • We are obliged to share confidential information without authorisation from the person who provided it, or to whom it relates, if it is in the public interest. That is when:

    • it is to prevent a crime from being committed or to intervene where one may have been, or to prevent harm to a child or adult; or
    • not sharing it could be worse than the outcome of having shared it.

    Please indicate below to confirm you understand the circumstances in which information may be shared without your consent. This will only be when it is a matter of safeguarding a child or vulnerable adult;

  • COLLECTION AND CONTACT DETAILS:

  • Please notify us of any changes to collection arrangements– in particular please notify us daily if your child is going home with another parent. We will enter the details into the collection book. No child will be released to an unauthorised person.

  • Emergency Contacts

  • In the event of an emergency where we cannot reach either parent/carer, please provide two additional emergency contacts;

  • Preschool session and fees are invoiced on a termly basis, sent via email. Invoices are normally raised at the start of term, although some may be delayed until County Council Headcount week if chargeable hours are unknown. Normal terms are 21 days.


    A copy of our billing policy and procedure is available on our website and a paper copy will be provided to you on request. By applying for a place for your child you are confirming that you have read and agree to comply with our billing policy. 

    *Since both parents who are resident with the child are jointly and severally liable for debts incurred by sending your child to preschool we will share information about your account with any other person having parental responsibility who was resident with the child at the time the debt was incurred. By signing that you will comply with the terms of our billing policy, you are agreeing to this sharing of information.


    All children are currently entitled to 15 hours per week of free early years education sessions through “Early Years Entitlement Funding.” This is available from the term after their third birthday until compulsory school start age, but there are cut-off dates specified nationally and by Devon County Council:

    30 hrs free childcare funding

    You may be able to get 30 hours free childcare if:

    ·         you live in England

    ·         your child is 3 to 4 years old

    Your eligibility depends on:

    ·         if you are working

    ·         your income (and your partner’s income, if you have one)

    ·         your child’s age and circumstances

    ·         your immigration status

     

    You can get 30 hours free childcare at the same time as claiming Universal Credit, tax credits, childcare vouchers or Tax-Free Childcare.

  • PRIVACY STATEMENT – GENERAL DATA PROTECTION REGULATION (GDPR)

  • We, Duchy Preschool and Playgroup (a Charitable Incorporated Organisation registered in England and Wales charity number 1157005, registered office Millway, Bradninch, Devon EX5 4NL) are the Data Controller for the purposes of the GDPR. We collect information from you and may receive information from other groups (such as other early years settings, schools and/or baby and toddler groups as authorised by you). We hold this personal data about you and your child(ren) and use it to:

    • Support the development and wellbeing of your child
    • Monitor and report on the progress of your child
    • Provide appropriate pastoral care to you and your child
    • Access free entitlement funding for your child
    • Manage invoicing and receivables for sessions we are permitted to charge for
    • Assess how well we are performing as an Early Years Setting
    • To contact you to inform you about your child, or to inform you about important Preschool information and events

    The information we hold includes:

    • Your contact details and details of other emergency contacts you give to us
    • Sessions booked for your child and attendance information
    • Information about your child's development 
    • Characteristics such as ethnic group
    • Special educational needs
    • Relevant medical information

    We will not give information about you or your child to anyone outside Duchy Preschool and Playgroup without your consent unless the law and our policies require and permit it. We are required to pass some of your information to the Local Authority (Devon County Council) and may be required to share information with other bodies. Information we hold about you will be kept secure, either by using locked storage for information on paper, or by using encrypted storage for electronic information.


    If you want to see a copy of the information that we hold and share on you or your child please ask the person responsible for data protection, our Manager Naomi Lee.

  • CONFIRMATION OF APPLICATION

  • I confirm my application for a place for my child * (name of child) at Duchy Preschool and Playgroup as detailed above.

  • I understand that the Breakfast Session starts at 08:00 and that Preschool Walking Bus in the morning will leave for The Duchy School promptly at 08:30. I understand that if I take breakfast sessions for my child I will be responsible for ensuring that my child has breakfast if I do not get my child to the Preschool until after 08:10, and also that Duchy Preschool and Playgroup will not be responsible for getting my child to school if I deliver them to the Preschool after the Walking Bus has departed. I also understand that no child will be allowed to join (or leave) the walking bus while it is “en route.” (Please also see the declaration on information sharing above)


    I understand that by signing this application I am agreeing to comply with the terms and conditions of Duchy Preschool and Playgroup, including the billing policy. I also confirm that I will read the Pre-school's policies on the website www.duchypreschool.org (or that I will ask for a copy of the policies to be loaned to me as appropriate). (Please note that all persons having parental responsibility should sign unless they are not normally resident with the child. Please also note that we cannot accept applications unless they are signed.)

  • Permission to photograph your child - Terms and Conditions

    1. Your consent is valid for the period your child attends the Duchy Preschool. If you have given appropriate consent, some images of your child may be used after this time in publicity material and on the Preschool website. If you wish these images to be withdrawn after your child leaves the Preschool please contact us in writing.
    2. Consent can be withdrawn at any time by contacting the Preschool in writing.
    3. The images we take will be of activities that show the Preschool and children in a positive light. Images may also be taken of children in our breakfast and after-school club.
    4. We may use group photographs or footage with very general labels e.g.
    ‘learning about nature’.
    5. If we use photographs of individual children on the website or in publicity material, we will not use the name of that child in the accompanying text or photo caption. If we name a child in the text, we will not use a photograph of that child to accompany the article.
    6. If a child has won an award and the parent would like the name of their child to
    accompany their picture we will obtain explicit permission from the parent before
    using the image.
    7. We will make every effort to ensure that we do not allow images to be taken of any children for whom we do not have permission or who are ‘at risk’ or disallowed from having their photographs taken for legal or social reasons.
    8. We will take all reasonable measures to ensure the images are used solely for the purposes for which they are intended and not passed on to any third party websites or publishers. However, we cannot guarantee this and take no responsibility for the way images are used by other websites or publishers or for any consequences arising from publication where images have been used without our consent.
