1. Original or copy of test/assessment report(s)* bearing recognized accreditation logo with certification number.
*[Test report validity period shall be 5 years for products listed under Product Listing Scheme with the issuance of product labels which can be displayed on the products.
Test report validity shall be 10 years for products listed under Product Listing Scheme with the issuance of declaration of conformity(DOC). ]
2. Product specification (e.g. Component list, material list, product brochure, drawing, etc).
3. Original or copy of latest information from the Accounting and Corporate Regulatory Authority (ACRA).
4. A copy of contract agreement between COC's Applicant and the designated installer.
5. Original or copy of letter from manufacturer informing the relationship between the manufacturer & COC's Applicant including role of the COC's Applicant concerning the product.
6. Original or copy of the authorization letter from the owner of test report (if the applicant is not the owner of the test report) confirming that the applicant is authorized to use the test report for application.
7. Copy of Certificate(s) to declared certification (e.g. ISO 9001, 14001, 18001, etc).
8. A copy of last COC from other certification body (ONLY applicable for Transfer Case).
