Admissions Interest Form
The Admissions Interest Form asks for general information about your child. If you have academic, diagnostic, and/or neuropsychological testing available, we ask that you please upload it below or send it to our Diagnostician at admissions@thestanthonyschool.com. Upon submission, our team will carefully review your child’s information and any testing provided to determine whether our program may be appropriate for your child’s needs. Our Admissions Team will then follow up with next steps as soon as possible. Please note: This is the first step in our admissions process, and no payment is necessary to complete this form.
TEFA Funding/ Admissions Timeline: If your family has received TEFA funds and plans to use them toward tuition, please let us know below. We understand that these funds may come with tight deadlines, and we will do our best to help move your family through the admissions process as efficiently as possible.
