FREDERICK FAMILY DOG TRAINING THERAPY DOG TRAINING AGREEMENT
I/We hereinafter referred to as “Client”, have voluntarily employed Kellie Frederick/Frederick Family Dog Training, hereinafter referred to as “Frederick” to assist me in the training of my dog, hereinafter referred to as “Client’s Dog”. Client acknowledges that they are the sole owner of Client’s Dog. Client acknowledges that Frederick is not liable for any possible incidents or liability outside of training, and liability resides solely with Client during training, and upon completion of Program.
Provided Equipment/Certificates
Frederick will supply a letter and the Certificates the Client’s Dog earns during the Program
Frederick will supply one ID card at no charge to Client and Client's Dog. Each additional ID card, whether a replacement or additional card, is an additional cost, paid to Frederick directly
Frederick will supply one Gentle Leader or Easy Walk Harness, one dual handle leash, and one bandana. Additional, replacements, or extra equipment is Client responsibility.
Training Steps
1. The First Step is assessing your dog to see what basic behaviors he/she knows so we can prepare him/her to successful pass our two important tests.
2. The Second Step is passing the AKC CGC (Canine Good Citizen) Test and AKC Community Canine, AKC CGC, Urban, or equivalent Public Testing.
3. The Third Step is training specialty task(s) needed, as well as completing the requirements specified in the FFDT Therapy Dog Program.
4. You may refer to the paperwork given to you for details on any phases and/or steps.
Therapy Dog Program
To help ensure we are able to meet the individual needs of clients, we offer a “Pay as You Go” payment plan which allows you to being your Therapy Dog Program right away without the added stress of coming up with the entire lump sum. This involves making a one-time, non-refundable payment of $200 to be enrolled in our program. Dogs must be evaluated and approved by one of our professionals to see if they would be a good fit for Therapy dog work. (See Step 1 above). Fee of $20 for Frederick to administer the AKC CGC Test is waived if enrolled in Therapy Dog Program. This does not include any fees for AKC CGC Titling with the American Kennel Club, itself. Each private lesson thereafter is $140.00, due in advance of each private training lesson. Our program offers additional benefits such as a custom approach to your specific dog, making the number of lessons needed, the things that are worked on unique to Client and Client’s Dog circumstances. Please note that the required minimum number of private lessons for Therapy Dogs is four (see below).

All potential Therapy dog(s) will be evaluated during our first step to see if they qualify for our program. Our professional trainer will make this evaluation before offering you a place in our program. Our trainers reserve the right to dismiss a dog from the program if there are signs or demonstrations of aggression towards a human or another dog. This is due to the fact that working dogs must have emotional stability and control and aggression is not tolerated at any time through the program. Enrollment fee of $200, as well as any payments made toward training, is non-refundable, even if a dog is dismissed from our program.
Client and Client’s Dog will complete minimum four successful working lessons/visits (the fore mentioned minimum four required lessons) with trainer present. Client and Client’s Dog will log minimum of twenty-five successful public outings of various locations. Public visits MUST be documented with provided Therapy Dog Visit Verification Form. The twenty-five public visits cannot be documented until the Client and Client’s Dog has been trained in basic obedience and is able to pass the Canine Good Citizen. Client and Client’s Dog will pass all requirements of Therapy Dog Program before graduating and completing our program.
Client and Client's Dog will adhere to Frederick Family Dog Training LLC's policies and procedures as outlined on their website, as well as in documentation given, and in the social media group platform, Facebook.
Client understands that to remain status as an Active Therapy Dog within the FFDT Therapy Dog Program, they must complete the requirements as stated in agreement and paperwork given, log a minimum of one visit every six months after graduation, and pay the $40 annual fee that includes the annual observation lesson, for the duration of the therapy dog’s working life. Client acknowledges that they may request, in writing, at any time, to be removed from Active Status, either for health reasons, personal reasons, dismissal, retirement, etc. and they will be removed from Active listing in the FFDT Therapy Program.

Client understands that Frederick will work directly with Client and Client’s Dog to impart contemporary animal behavior knowledge that best fits our needs, and that successful companion pet programs depend on a combination of learned skills on the part of the pet and owner.
Behavior is not static; an animal will not continue to perform even trained behaviors without ongoing practice. Especially involving any types of aggression or anxiety, although behavior may be modified, the dog is never considered “cured”. A pet’s behavior is ultimately the owner’s responsibility. Frederick will make every reasonable effort to help us attain goals but makes no guarantees of performance on the part of Client or pet as a result of providing professional animal behavior consultation.
Client acknowledges that Frederick adheres to the AKC guidelines for Canine Good Citizen, Therapy Dogs International, and other organizations to be granted the proper certification, but maintains their status as a private trainer, issuing their own certificates. Client acknowledges that by signing this contract that client as well will uphold these guidelines throughout the Therapy Dog’s life. Client acknowledges that obedience training and behavior modification is an activity that may cause damage or result in injury to Client’s dog, other dogs, people, and property. Client will assume full financial and moral responsibility for the actions of Client’s dog. Client acknowledges Frederick's recommendation to obtain their own private liability insurance for their therapy dog.
Client will also assume full financial responsibility for any tickets Frederick incurs if Client’s dog is not legally spayed/neutered and/or licensed by the city, as pursuant to local law.
Client further acknowledges that Client’s dog may be exposed to a variety of environmental conditions, which include, but are not limited to, interaction with people and other animals, exposure to adverse weather, and exposure to areas with crowds and all types of traffic. Client assumes all risks associated therein.
Client and Frederick agree to settle, by binding arbitration, any dispute arising from any injury or damage done to or by Client’s dog while Client’s dog is in the custody and control of Frederick or which is the result of gross negligence by Frederick. Client forever releases Frederick from all liability and demands, which occur following binding arbitration for any injury or damage done to or by Client’s dog that was the subject of the arbitration.
Client and Frederick hereby agree to mediate and/or arbitrate any misunderstanding that may arise pursuant to the terms contained herein. In any action or proceeding arising out of this Agreement, the prevailing party shall be entitled to reimbursement of costs and legal fees.
(III)
This contract for Training Services supersedes all other Agreements, written or oral, previously made between Client and Frederick.
This Agreement shall be governed by and construed in accordance with the laws of the State of Iowa.
