Release Form & Waiver of Liability
I, as parent or guradian of the minor child identified below am aware that participation in circle in wooded areas, social contact, animal contact, sports or physical exercise (such as yoga), exposure to COVID-19, may result in accident or injury, and hereby assume the risk connected with the participation in a gathering with the facilitator(s) and volunteer(s) of Sacral Wisdom & Beyond at any location in the Los Angeles County.
I affirm that my child is in good health and suffers from no physical impairment that would limit their participation in such a circle.
I acknowledge that Sacral Wisdom & Beyond will not render any medical services including medical diagnosis of my child's physical condition.
I grant permission to Sacral Wisdom & Beyond to take and use photographs and/or video of me in publications, news releases, online, and in other communications related to Sacral Wisdom & Beyond.
I specifically agree that the facilitator(s) and voluteer(s) of Sacral Wisdom & Beyond, the city of Los Angeles, the city of Culver City, and LA County shall not be liable for any claim, demand, cause of action of any kind whatsoever for, or on account of any kind whatsoever for, or on account of personal injury, property damage, or loss of any kind resulting from, or related to, my and/or my child's use of the facilities or participation in any exercise, or activity, and I agree to hold Sacral Wisdom & Beyond, the city of Los Angeles, and the city of Culver City harmless from same.
I affirm all of the above stipulation for myself as well.
I have read the above release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.
